Terms Of Service

A1 MEDICAL SERVICES, LLC is a private pay service. Blood draw and specimen collection services are provided and billed
by A1 MEDICAL SERVICES, LLC. We do not bill your insurance for our service fee. Payments for our services are made
directly to A1 MEDICAL SERVICES, LLC and must be made before services are rendered unless stated otherwise. Please be
advised that all laboratory tests will be billed to your insurance company by the resulting lab. A1 MEDICAL SERVICES, LLC is
not responsible for any outstanding balance or charges you may incur. For any billing questions or concerns contact your
insurance company and/or resulting laboratory billing department listed on your statement. 

 

A1 MEDICAL SERVICE, LLC is an independent mobile phlebotomy service offered as a convenience to patients. A1 MEDICAL
SERVICES, LLC will come to your home, workplace, or facility for lab draws and specimen collection. obtain your blood
sample(s) by venipuncture or capillary and properly deliver or ship it to the laboratory.
                              
A1 MEDICAL SERVICES, LLC is not in any way an acting diagnostic laboratory or physician’s office. Therefore we are not
authorized to diagnose, treat or give medical advice to any person. 
 
 
A1 MEDICAL SERVICES, LLC is not responsible for any ordering inaccuracies or laboratory testing errors. Please contact
your physician’s office or laboratory for correction. In the event that a recollect is needed due to an error not made by A1
MEDICAL SERVICES, LLC there will be a recollect charge. Any recollection error caused by A1 MEDICAL SERVICES, LLC
will be recollected at no charge to the patient.

A1 MEDICAL SERVICES, LLC is in full awareness of patient’s privacy, and we assure that all information will be kept
confidential. Under no circumstances will your information be shared with any unauthorized personnel. A1 MEDICAL
SERVICES, LLC does not receive nor have access to your laboratory results; you must contact the ordering physician or
resulting laboratory.

A1 MEDICAL SERVICES, LLC will need the following before the blood collection process. Original lab order, insurance
card, photo ID (will be checked in person) and service fee. We do not accept verbal lab orders. If you are self-paying the service
fee and laboratory fee must be paid in full on or before services are rendered. To better prepare for your appointment, please have
all documents prior to your scheduled appointment.

A1 MEDICAL SERVICES, LLC is a convenient blood draw service provided to the healthcare community. Although each
patient is very important to us, our scheduled appointments are a priority. We encourage you to take advantage of our online
booking to guarantee your blood collection appointment. You will receive a courtesy call or text the day of your appointment
when your technician is en route. For any reason you need to reschedule your appointment you may contact A1 MEDICAL

Please be advised that a same day cancellation will result in a $15 charge. There is a $25 charge if our technician arrives at your
destination and you are not there or you reschedule. Due to our technician's schedule they are not allowed to wait around.
